- A new cough, fever, sore throat, shortness of breath, or muscle aches not attributed to another health condition?
- Have you had contact or live with anyone with a presumed or confirmed case of COVID-19?
If the answer is “YES” to any of these questions, we will ask that you remain home and only return to VNS with a doctor’s note.
By adding your initials in lieu of signing, you are stating that to the best of your knowledge, your child is free of all the above symptoms and is healthy to be at school.
